IRIS SUPPORT BROKER SERVICES

WHO WE SERVE 

Avenues to Community Support Broker services are available to people with disabilities and older adults who need assistance in self-directing their services in IRIS. Support Broker services are also available to people through private pay arrangements. Avenues Support Brokers are not affiliated with other service providers, minimizing any conflict of interest.

Avenues to Community Support Broker services are available to people with disabilities and older adults who need assistance in self-directing their services in IRIS. Support Broker services are also available to people through private pay arrangements. Avenues Support Brokers are not affiliated with other service providers, minimizing any conflict of interest.

 

SERVICES WE PROVIDE

The services provided will be based on the long-term care outcomes that demonstrate the need for Support Broker services. These services cannot conflict with the IRIS consultant’s role, although some services can complement the help the IRIS consultant provides. There is no minimum number of hours of service that can be requested and services can begin at any time. Services can also be provided on a short-term basis.

The following is a list of examples of services an Avenues to Community Support Broker can provide:

  • Help review and monitor your IRIS spending to ensure you stay on budget.
  • Help you understand Social Security benefits and help you communicate with Social Security when needed.
  • Help you manage your own personal finances and refer you for trust and benefits counseling.
  • Help you complete your Medicaid and FoodShare reviews.
  • Attend medical appointments with you if needed.
  • Assist with communication between medical providers and service providers.
  • Help you follow through with your doctor’s recommendations.
  • Help you get the right specialized medical equipment and supplies you need.
  • Participate in transition planning from high school to adult services.
  • Participate in discharge planning from hospitals or nursing homes back home.
  • Help with planning when you move from your family home to a home of your own.
  • Assist with end of life and funeral planning.
  • Help you identify your skills and talents  which you can use to become more  involved in your community.
  • Help you identify social, recreational, and educational opportunities to increase community involvement.
  • Assist with finding appropriate, affordable housing if a housing counselor is unavailable.
  • Assist with housemate matching.
  • Identify employment opportunities and assist with the referral process for these opportunities.
  • Help recruit workers you wish to hire as caregivers.
  • Help write job descriptions.
  • Help pre-screen potential applicants.
